A Vision of Excellence: Urban Health Plan's Journey to Transform Community Healthcare in New York

Recently, we had the pleasure of diving into the world of Urban Health Plan (UHP) with Matt Siebel, their dedicated in-house attorney. Since its establishment in 1974, UHP has been a guiding light in community health care. Today, it is one of the largest FQHC health system in New York state, focusing on quality, equity, patient access, and addressing the social drivers of health.

Serving as the newly appointed Board Chair of the National Association of Community Health Centers, Paloma Izquierdo-Hernandez, CEO of UHP, is a true visionary who epitomizes the definition of leadership. Paloma, who was raised in the Bronx, anchors the organization's day-to-day work with the values that underscore the diversity, equity, and inclusion she champions.

UHP's new 50,000-square-foot specialty care facility and innovative virtual care programs highlight its dedication to evolving healthcare practices. With success stories that include serving 83,000 patients annually and receiving the 2023 HRSA Gold Health Center Quality Leader Award, their holistic care model shines.

Our recent partnership with UHP played a crucial role in helping them expand services at Plaza del Sol Family Health Center, located at 37-12 108th Street, Corona (Queens County), NY, which will soon begin construction at 345 Soundview Avenue in the Bronx.  The expansion at Plaza Del Sol will take the site from 14,600 to 22,992 square feet, allowing UHP to add 13 examination rooms and additional services! The Logistics Center will support and improve the efficiency of health and wellness services within Urban Health Plan's 26 healthcare sites.
